Malaria vaccine trial raises hope

Researchers are to expand a clinical trial of a new malaria vaccine after promising results in a preliminary study in Burkina Faso.

The trial was designed to test safety, but researchers found that vaccinated children had high levels of protection.

Described as a "most encouraging" result, a larger study involving 800 children is now to take place in Mali.

The scientists involved say they are hopeful that the vaccine will ultimately be very cheap to produce.

Around a hundred different malaria vaccine candidates have been developed to date but the MSP3 vaccine tested in Burkina Faso is only the second one to show a substantial level of protection against the illness.

The randomised, double blind study involved 45 children. It set out to test the safety of the vaccine but this follow up study found that children who received it had an incidence of the disease three to four times lower than children who did not.

Initially the children were split into three groups, with two of them receiving the experimental malaria vaccine developed by Dr Pierre Druilhe at the Pasteur Institute in Paris.

"Those two groups had very similar types of immune response, elicited by the vaccine, and the protection is almost identical, so it reinforces the confidence despite the fact that we are still dealing with a small group," he said.

The vaccine is based on the fact that some adults in Africa acquire immunity because they are constantly exposed to the disease.

Early days

Dr Druilhe and his team discovered a key protein, MSP3, which provokes the body into producing antibodies that kill the parasite.

He said the protein is unique as it does not change much between different strains of the plasmodium parasite that causes malaria. This is believed to be a critical factor in developing an efficient vaccine.

He added: "We performed a large number of epidemiological studies that confirm that there was an association between that vaccine candidate and acquired protection, so when you immunise with this molecule you indeed induce protection."

Another scientist involved with the Burkina Faso study was Dr Louis Miller, the former head of the Malaria Vaccine Branch of the US National Institutes of Health.

He said: "I was always in favour of this approach as it offered a chance in a field with few successes. I found the results of this preliminary study in Burkina Faso to be most encouraging."

High transmission

Encouraged by the early results, Dr Druilhe said the trial has now been expanded to 800 children in Mali. But he remains cautious.

"There have been too many claims of effective vaccines so we have to remain very cautious. It has to be confirmed and we have started on work to do that confirmation. Essentially the trial in Mali is about 20 times larger, in extremely high transmission conditions, so it should yield very clear cut results - this will be black and white."

The other vaccine candidate that has shown success against malaria is called RTS, S. It has been funded by the Bill and Melinda Gates Foundation and is set to go into production with pharmaceutical giant, GlaxoSmithKline.

But there are concerns that it could be expensive, especially for people in Africa and other regions affected by the disease.

Dr Druilhe says his vaccine could be a lot cheaper - perhaps half a dollar or less a bottle.

The results of the Burkina Faso trial were published in The New England Journal of Medicine.

Aamulehdestä hyviä ohjeita kesäruoan tekoon

Työtehoseura muistuttaa kesäjuhlien järjestäjiä olemaan tarkkana ruokatarjoilujen kanssa. Pihajuhlat ovat pilaantuvien ruokien kannalta erityisen haasteellisia.

Työtehoseuran vinkit:

Ruokatarvikkeiden ostaminen ja kuljetus

– Ennen ostoksille lähtöä säädä jääkaapin termostaatti kylmemmälle tai käytä pikajäähdytystoimintoa. Näin ostokset jäähtyvät kuljetuksen jälkeen nopeasti.

– Osta tuoreita elintarvikkeita ja lue pakkausmerkinnät. Tarkista helposti pilaantuvien elintarvikkeiden viimeinen käyttöpäivä.

– Kerää kylmänä säilytettävät tuotteet ostoskoriin tai kylmälaukkuun viimeisinä.

– Kuljeta kylmänä säilytettävät ruoat kotiin nopeasti ja mielellään kylmälaukussa.

– Matkustaessasi osta elintarvikkeet lähellä määränpäätä. Käytä mahdollisuuksien mukaan sähköllä tai kaasulla toimivaa matkakylmälaukkua.

Kylmäsäilytys

– Tarkista jääkaapin ja pakastimen lämpötilat. Jääkaapin sopiva lämpötila on 4–6 astetta ja pakastimen -18 astetta. Helposti pilaantuvat kalatuotteet vaativat 0–3 asteen lämpötilan säilyäkseen viimeiseen käyttöpäivään asti.

– Laita ruoat heti kylmälaitteeseen. Älä täytä jääkaappia niin täyteen, että ilmankierto laitteen sisällä heikkenee.

– Älä säilytä ruokia avonaisissa astioissa, vaan peitä tai pakkaa ne kunnolla.

– Pakastaessasi elintarvikkeita, älä ylitä laitteen pakastuskykyä.

– Käytä pakasteet vuorokauden sisällä sulattamisesta, sillä ne pilaantuvat nopeammin kuin vastaavat tuoretuotteet.

– Älä pakasta sulaneita pakasteita uudelleen, ellet kypsennä niitä välillä.

– Noudata säilytyslämpötiloista ja -ajoista annettuja pakkausmerkintäohjeita. Muista, että viimeinen käyttöpäivä tai parasta ennen -päiväys koskevat vain avaamattomia tuotteita!

Esikäsittely ja valmistus

– Pese kädet ennen ruoanvalmistusta ja aina, kun siirryt käsittelemään erilaista raaka-ainetta.

– Pese salaattiainekset ja kuori juurekset, jos et saa niitä pesemällä puhtaiksi.

– Käytä eri elintarvikkeille, erityisesti raa’alle lihalle, omaa leikkuulautaansa.

– Käytä puhtaita työvälineitä ja -astioita.

– Kuumenna lämpimänä tarjottavat ruoat kauttaaltaan. Kypsennä etenkin siipikarja ja sianliha sekä jauheliha läpikypsäksi.

– Vaihda keittiöpyyhettä usein ja pyyhi sillä ainoastaan liettä ja keittiön työpöytiä.

– Älä valmista ruokaa muille, jos olet sairas.

– Älä päästä lemmikkieläimiä työpöydille.

Tarjoilu

– Tuo kylmät ruoat tarjolle kylminä ja lämpimät ruoat kuumina, yli 60 asteisina.

– Nosta maidot ja vastaavat ruoat heti ruokailun päätyttyä takaisin kylmään.

– Lämmitä ruokaa vain kerran, sillä mikrobit kasvavat jokaisella jäähdytyskerralla ja uudelleen lämmittäminen heikentää ravintosisältöä.

– Älä nosta kaikkea ruokaa heti tarjolle, vaan täydennä tarjoilupöytää tarpeen mukaan. Tarjoiluastioiden peittäminen kansilla auttaa ruokien kylmänä tai lämpimänä pysymistä.

– Ruoanvalmistuksessa voi käyttää jäisiä elintarvikkeita, jotka lisätään ruokaan vasta tarjolle vietäessä. Esimerkiksi salaattiin voi lisätä jäiset katkaravut tai pakastevihannekset vasta hetkeä ennen tarjoilua.

Kylmäsäilytys pihajuhlissa

– Ruokien tarjoilupiste kannattaa sijoittaa varjoisaan, katettuun paikkaan.

– Ruoat säilyvät hetken myös kylmälaukuissa, joihin voi laittaa muutamia kylmävaraajia kylmyyttä lisäämään. Huopa kylmälaukun ympärillä tuo lisäeristystä. Hätätapauksessa kylmätilan voi tehdä paksulla sanomalehtikerroksella vuoratusta pahvilaatikosta.

– Pihassa oleva maakellari tai kaivo, jonne tavarat voi hetkeksi laskea ämpärissä, ovat toimivia ratkaisuja.

– Kylmyyttä pitää hetken myös kylmällä vedellä kasteltu pyyheliina.

Adrenomedullin is a peptide associated with pheochromocytoma- a tumour arising from adrenal medulla. It was discovered in 1993.[1]

Adrenomedullin (AM) is a ubiquitously expressed peptide initially isolated from phaechromyctoma in 1993 (Kitamura et al., 1998). Since its first report, studies examining the effects of adrenomedullin have mushroomed, highlighting its role in a number of diseases. Recently a second peptide AM2 has been recognised, exhibiting similar functions (Fujisawa et al., 2004).

[edit] The Adrenomedullin Peptide

The human AM gene is localized to a single locus on Chromosome 11 with 4 exons and 3 introns. The AM gene initially codes for a 185-amino acid precursor peptide [3], that can be differentially excised to form a number of peptides, including an inactive 53-amino acid AM, e PAMP, adrenotensin and AM95-146. Mature human AM is activated to form a 52 amino acid, 6-amino acid ring, that shares moderate structural similarity to the calcitonin family of regulatory peptides (calcitonin, CGRP and amylin). Circulating AM consists of both amidated (mature) and the glycated form (inactive, with the latter comprising the major form (85%) [5]. The measured to have a plasma half-life of 22min, mean clearance rate of 274 mL/kg/min, and apparent volume of distribution of 880+/- 150 mL/kg (Meeran et al., 1997). Mature AM is metabolised via aminopeptidase action.

[edit] Receptors of Adrenomedullin

At present AM is believed to function through combinations of the calcitonin receptor like receptor (CALCRL) and receptor activity-modifying proteins (RAMP2) complexes, as well as CGRP receptors. It is worth noting that unlike the classical one ligand-one receptor notion of receptor signalling, the interaction of both Calcitonin receptor-like (CALCRL) and RAMP ( Receptor activity-modifying protein) at the membrane, is required for AM to mediate its action. The outcome of AM stimulation of its receptor is the cellular production of both cyclic AMP (cAMP) and nitric oxide production. Some may find the production of these inside the cell to be at odds, since often they have opposing effects, but as yet, the timing of these effects remains to be studied.[citation needed]

[edit] The physiological functions of Adrenomedullin

AM was initially identified as a vasodilator, some have cited this as the most potent endogenous vasodilatory peptide found in the body (Cockcroft et al., 1997). Differences in opinion regarding the ability of AM to relax vascular tone arises from the differences in the model system used (Hamid and Baxter, 2005). Other effects of AM include increasing the tolerance of cells to oxidative stress and hypoxic injury and angiogenesis. AM is seen as a positive influence in diseases such as hypertension, myocardial infarction, chronic obstructive pulmonary disease and other cardiovascular diseases, whereas it can be seen as a negative factor in potentiating the potential of cancerous cells to extend their blood supply and cause cell proliferation.

Abstract

Adrenomedullin (AM) is a vasodilator peptide having a wide range of biological actions such as reduction of oxidative stress and inhibition of endothelial cell apoptosis. The AM gene is expressed in vascular walls, and AM was found to be secreted from cultured vascular endothelial cells, smooth muscle cells, and adventitial fibroblasts. Plasma AM levels in patients with arteriosclerotic vascular diseases are elevated in possible association with the severity of the disease. When administered over a relatively short period, AM dilates blood vessels via an endothelium-dependent or independent mechanism. Experiments in vitro have shown that AM exerts multiple actions on cultured vascular cells, which are mostly protective or inhibitory against vascular damage and progression of arteriosclerosis. Either prolonged infusion or overexpression of AM suppressed intimal thickening, fatty streak formation, and perivascular hyperplasia in rodent models for vascular remodeling or atherosclerosis. Intimal thickening induced by periarterial cuff was more severe in AM gene-knockout mice than their littermates, suggesting a protective role for endogenous AM. Moreover, AM has recently been suggested to possess angiogenetic properties. Collectively, a body of evidence suggests that AM participates in the mechanism against progression of vascular damage and remodeling, thereby alleviating the ischemia of tissues and organs.

Abstract

The hypotensive and vasorelaxing peptides adrenomedullin (AM) and its gene-related peptide, proadrenomedullin N-terminal 20 peptide (PAMP), were found to be distributed on the surface of the colonic mucosa. AM and PAMP showed dose-dependent antimicrobial activity against E. coli. The results suggest that the novel vasoactive peptides AM and PAMP play an important role in mucosal defence.
