Challenges in the Diagnosis and Treatment of Lyme Disease.
Schoen RT. Curr Rheumatol Rep. 2020 Jan 7;22(1):3. doi: 10.1007/s11926-019-0857-2. PMID: 31912251 Review.
The purpose of this review is to examine advances in the diagnosis and treatment of Lyme disease, as well as ongoing management challenges. RECENT FINDINGS: Recent findings: It is useful to recognize that Lyme disease occurs in stages, with early- and late-stage disease. Clinical expression is in part determined by Borrelial variability. For example, some strains of Borrelia burgdorferi, the causative organism in North America, are particularly arthritogenic. Most patients with early Lyme disease can be cured with a single course of oral antibiotic therapy, in contrast to some patients with Lyme arthritis, a late-stage manifestation, who are more antibiotic refractory and require other treatment strategies. Successful treatment of Lyme disease begins with successful diagnosis and with an understanding of the emergence, clinical features, and impact of Lyme disease over the past half century.

Abstract
Lyme disease is now the most frequently reported vector-borne disease in the United States. The highest incidence is in children aged 5 to 9 years with a male predominance. The most common manifestation, erythema migrans, is sometimes not recognized, leading to risk of complications. Testing for Lyme disease should only be done if there is a consistent clinical syndrome with exposure in a Lyme-endemic area. Most forms of Lyme disease are successfully treated with short courses of oral therapy. Prevention and management of tick bites is important. Abstract uman granulocytic anaplasmosis (HGA) is a bacterial infection caused by Anaplasma phagocytophilum and transmitted by the bite of the black-legged (deer tick) in North America. Its incidence is increasing. HGA can be transmitted after 24 to 48 hours of tick attachment. The incubation period is 5 to 14 days after a tick bite. Symptoms include fever, chills, headache, and myalgia. Complications include shock, organ dysfunction, and death. Mortality is less than 1% with appropriate treatment. Doxycycline is first line treatment for all ages. Start it empirically if symptoms and risk factors suggest HGA. PCR is the confirmatory test of choice. Keywords: Anaplasmosis; Deer tick; Doxycycline; Ixodes; Leukopenia; Thrombocytopenia; Tick-borne.Copyright © 2022 Elsevier Inc. All rights reserved.

Anaplasma phagocytophilum is a zoonotic obligate intracellular bacterium known to be transmitted by ticks belonging to the Ixodes persulcatus complex. ...Here, we review the current knowledge and future perspectives regarding A. phagocytophilum epidemiology a … This bacterium can infect several mammalian species, and is known to cause diseases with variable symptoms in many domestic animals. Specifically, it is the causative agent of tick-borne fever (TBF), a disease of important economic impact in European domestic ruminants, and human granulocytic anaplasmosis (HGA), an emerging zoonotic disease in Asia, USA and Europe. A. phagocytophilum epidemiological cycles are complex and involve different ecotypes, vectors, and mammalian host species. Moreover, the epidemiology of A. phagocytophilum infection differs greatly between Europe and the USA. These different epidemiological contexts are associated with considerable variations in bacterial strains. Until recently, few A. phagocytophilum molecular typing tools were available, generating difficulties in completely elucidating the epidemiological cycles of this bacterium. Over the last few years, many A. phagocytophilum typing techniques have been developed, permitting in-depth epidemiological exploration. Here, we review the current knowledge and future perspectives regarding A. phagocytophilum epidemiology and phylogeny, and then focus on the molecular typing tools available for studying A. phagocytophilum genetic diversity.

Iatrogenic Botulism- European Region

24 March 2023

Situation at a glance

On 7 March 2023, the National IHR Focal Point (NFP) for Germany notified WHO of five cases of iatrogenic botulism in individuals who underwent medical procedures with the injection of botulinum neurotoxin type A (BoNT/A) in health institutions in Türkiye. As of 17 March 2023, a total of 71 cases were reported in four countries in the European Region, mainly linked to two hospitals in different locations in Türkiye. The products used for the treatment were seized and taken for examination and evaluation by the Turkish Medicines and Medical Devices Agency. No new symptomatic cases have been reported since 8 March 2023.  Outbreaks of botulism are very rare and can be associated with a natural, accidental, or potentially deliberate source of infection.

Description of the situation On 7 March 2023, the National IHR Focal Point (NFP) for Germany notified WHO of five cases of iatrogenic botulism in individuals who underwent medical procedures with the injection of botulinum neurotoxin type A (BoNT/A) in health institutions in Türkiye.  As of 17 March 2023, a total of 71 clinical botulism cases were reported from Türkiye (53 cases), Germany (16 cases), Austria (one case) and Switzerland (one case) linked to the aforementioned medical interventions performed in Türkiye between 22 February and 1 March 2023.All cases are adults; most cases are middle-aged women. Among the 69 cases for which treatment location information is known, two private hospitals in two locations in Türkiye were identified, with 66 cases linked to one hospital and three cases to another hospital. The clinical presentation of the cases ranged from mild to severe. Clinical signs of BoNT intoxication were observed (including fatigue, headache, blurred and/or double vision, dizziness, ptosis, dysphagia, dyspnea, neck weakness, generalized muscle weakness, and swollen tongue). Several cases were hospitalized. Some cases were treated with botulinum antitoxin. At least five cases were admitted to intensive care units. There are no reported deaths. Investigations carried out by Turkish authorities reported that licensed BoNT products were administered for a different purpose other than for which the products were approved (off-label use). The relevant departments of both hospitals had their activities suspended on 1 March 2023, and investigations have been launched against the parties involved.  The products used for the treatment were seized and taken for examination and evaluation by the Turkish Medicines and Medical Devices Agency. No new symptomatic cases have been reported since 8 March 2023, which was the date of symptom onset for the last reported case.

Public health response
The Turkish Ministry of Health is working with the Türkish Medicines and Medical Devices Agency and the Turkish National Poison Solidarity Center to investigate this event. The Turkish General Directorate of Public Health, Department of Communicable Diseases and Early Warning. is conducting an epidemiological investigation with provincial health centres and related departments. The relevant units of the Ministry of Health and the Turkish Police Service urgently inspected the two hospitals mentioned above. The investigation is currently ongoing to determine if the cause of the intoxication is due to product quality/safety, medical error, or an overdose. WHO is collaborating with the European Centre for Disease Prevention and Control (ECDC), and the affected IHR State Parties, regarding investigation, risk assessment, information sharing and response for this event. The Turkish Medicines and Medical Devices Agency published an announcement on its website on 10 March 2023. Similarly, the ECDC announced the current situation regarding this event in their publicly-available Communicable Disease Threats Report on 10 March 2023, and a news story was published on the ECDC website on 14 March 2023.

WHO risk assessment
Botulinum neurotoxins are dangerous toxins produced by several species of the genus Clostridium (e.g., C. botulinum, C. baratii, C. butyricum, and C. sporogenes) under anaerobic conditions. Botulinum neurotoxins are one of the most lethal substances known and occur in eight serotypes (A to H). Botulinum neurotoxins block nerve functions and can lead to muscular, including respiratory, paralysis. Human botulism is a naturally occurring rare disease and may refer to foodborne botulism, infant botulism, wound botulism, and inhalation botulism.

Clostridium botulinum culture is used to produce pharmaceutical botulinum toxin-containing products. These pharmaceutical products employ the purified and heavily diluted botulinum neurotoxin (type A or type B, depending on the indication) injected for clinical use for the treatment of more than 20 neurological and non-neurological disorders, as well as for cosmetic use. Treatment is administered in the medical setting, tailored according to the needs of the patient and desired treatment effect.

The symptoms of botulism can be very severe, requiring intensive-care treatment as well as the administration of specific anti-toxins. Even when such treatments are available, severe botulism cases require supportive treatment, including mechanical ventilation. Complete recovery usually takes weeks to months.

This current event is possibly linked to a specific procedure and/or a medical product in at least two healthcare settings. Further investigations are ongoing regarding case ascertainment and notification. The investigation remains open for other possible hypotheses of intoxication. The respective departments in the two clinics where the procedures were undertaken have been closed since 1 March, with no further cases reported.

There are several clinical indications for the use of botulinum neurotoxin; however, its off-label use in bariatric cases has increased in many countries. Depending on the product used, a warning of toxic effects may be insufficiently worded, and the risks not adequately addressed in the patient consent process.

WHO advice

Botulism outbreaks are rare, but outbreaks require rapid recognition to identify the source of the disease, distinguish between types of outbreaks (natural, accidental or potentially deliberate), prevent additional cases, and effectively administer treatment to affected patients.

Successful treatment depends significantly on early diagnosis and the rapid administration of the botulinum antitoxins. Iatrogenic botulism should be suspected if a patient has recently received botulinum neurotoxin, and clinicians should remain mindful of the risk of systemic botulism with BoNT therapy. The diagnostic detection of BoNT/A in patients suffering from iatrogenic botulism might be highly challenging due to the extremely low dose of toxin present and the potentially suboptimal timepoint of serum sampling. So far, very few detection methods have been shown to be validated on clinical specimens with a sensitivity better than the classical mouse bioassay, which is still regarded as a reference for BoNT detection in clinical samples. At this time, it cannot be confirmed if the reported cases of botulism intoxication are due to product quality/safety, medical error, or an overdose.

Although the source of the outbreak may have been removed, WHO Member States need to remain vigilant. It is therefore critically important for the IHR State Parties to identify any additional possible case, to establish the cause, and to determine if the product used in the procedure could be substandard or falsified. Investigation of these cases will assist in rapid containment at the disease source to prevent additional possible cases.

WHO does not recommend any travel and/or trade restrictions to Türkiye based on the information available for this event.

Clostridium species

MICROBIOLOGY

  • Clostridia produce the largest number of toxins of any bacterial class.
  • The common source of entry in humans is the gastrointestinal tract or trauma.
  • Obligate anaerobic bacteria with unusual features: spore formation, toxin production and normally reside in the environment or the gastrointestinal tract of humans and animals. Dominant form in the human intestinal flora.
  • Belongs to phylum Firmicutes, which is the dominant component of the fecal microbiome. Clostridia spp. account for 50% or about 1013/gm stool.
  • Species-specific toxin expression:
    • Botulism (C. botulinum)
    • Colitis (C. difficile)
    • Spore-forming anaerobe, Gram-positive bacillus [Fig 1].
      • Found in human and animal feces, also in water and soils.
      • Reclassified as molecular sequencing suggests that the organism should be in the Peptostreptococcaceae family and termed Peptoclostridium.
        • Clostridioides difficile name selected to differentiate from Clostridiaspp., which are not related but allowing for less clinical confusion moving from the long-standing terminology of Clostridium difficile[12].
    • Produces toxins A and B that cause colitis in humans.
    • Occasionally grown in anaerobic cultures, rarely a cause of infection other than colitis.
    • Tetanus (C. tetani)
      • Clostridium tetani is a Gram-positive bacillus, obligate anaerobe.
        • If culture enters the endospore stage, it may have classic tennis racquet morphology [Fig 1].
      • Spores are widespread in soil and animal feces.
      • The rod-shaped appearance on a Gram stain is often likened to drumsticks or tennis rackets.
      • Toxin, tetanospasmin, causes tetanus: unopposed muscle spasm and contraction due to interference of toxin with neurotransmitters (including GABA and glycine).
    • Septic abortion (C. sordellii)
    • Gas gangrene (C. perfringens, C. novyi, C. septicum, C. sordellii, C. histolyticum)

Common clostridial species causing gas gangrene:

    • Clostridium perfringens
      • Most commonly identified organism.
      • Produces > 20 exotoxins, including lethal Alpha and Beta toxins that cause necrosis and hemolysis.
    • Clostridium novyi
    • Clostridium septicum
    • Clostridium histolyticum
    • Clostridium sordellii (seen with toxic shock after childbirth, users of contaminated heroin injections and patients with malignancy)
      • Review of spontaneous C. sordelli-associated gas gangrene found that known or occult malignancy was seen in 71% of the 94 patients in the literature; overall mortality was 67%[3].
    • Clostridial food poisoning (C. perfringens)
  • Non-toxigenic strains are often components of mixed infections or bacteremia; pathogenic role often unclear.

Aktiivista tuberkuloosista tuore tutkimus vuodelta 2022 . Vilkaisu flavivirukseen WNV.

( Tässä en ota  Covidin  aktuella taulukkoa. Se on WHO lähteessä. Mortaliteetti lähenee 7 miljoonaa, muta on jo laskevaa käyrää).

Esim. voisiko samanlaiset  terapiat antaa jollain tavalla  terveystuloksia - varsinkin Long-Covidissa? -esim  ravinto, hygieniset asunto-olot,   ravitseva ravinto, maitoproteiininkin  hyvä teho fosfolipidien  ja keuhkosuoja-aineiten antajana ja  vektoreiden välttö  zoonoositaudeissa.

Realiteetti , epidemiologinen nykytilanne maailmassa , morbiditeetti- ja mortaliteetti-lastien  erot  näiden tautien kesken. Haen joitain statistisia tietoja.

WHO:

TB is caused by bacteria (Mycobacterium tuberculosis) and it most often affects the lungs. TB is spread through the air when people with lung TB cough, sneeze or spit. A person needs to inhale only a few germs to become infected.

Every year, 10 million people fall ill with tuberculosis (TB). Despite being a preventable and curable disease, 1.5 million people die from TB each year – making it the world’s top infectious killer.

TB is the leading cause of death of people with HIV and also a major contributor to antimicrobial resistance.

Most of the people who fall ill with TB live in low- and middle-income countries, but TB is present all over the world. About half of all people with TB can be found in 8 countries: Bangladesh, China, India, Indonesia, Nigeria, Pakistan, Philippines and South Africa.

About a quarter of the global population is estimated to have been infected with TB bacteria, but most people will not go on to develop TB disease and some will clear the infection. Those who are infected but not (yet) ill with the disease cannot transmit it.

People infected with TB bacteria have a 5–10% lifetime risk of falling ill with TB. Those with compromised immune systems, such as people living with HIV, malnutrition or diabetes, or people who use tobacco, have a higher risk of falling ill.

West Nile Virus on flavivirusryhmää ja tulee linnuista   hyttysten puremien kautta.

 https://www.ecdc.europa.eu/en/west-nile-fever/facts

West Nile virus (WNV) infection is a mosquito-borne zoonosis that is endemo-epidemic in Europe. The disease affects countries in southern, eastern and western Europe. The virus is transmitted among birds via the bite of infected mosquitoes and incidentally humans and other mammals may become infected. About 80% of WNV infections in humans are asymptomatic. West Nile fever (WNF) is the most common clinical presentation. The elderly and immunocompromised persons are at higher risk of developing West Nile neuroinvasive disease (WNND). No specific prophylaxis or treatment exist against the disease in humans.

Osa Ukrainasta tulleista lapsista on kuitenkin erityisen alttiita infektiotaudeille esimerkiksi puutteellisen rokotussuojan takia.

Varsinkin Suomessa asia on olennainen, koska on  erittäin pitkä metsäraja  Venäjän metsiä vastaan, ja sieltä muutenkin  hiippailee yli  supikoiria, susia, kettuja, villikoiria  jne , koska eihän  metsän villieläimet rajoja  tunnista.

Ukrainasta pakolaisten mukana saapuvien lemmikkien pelätään muuttavan peruuttamattomasti suomalaisen marjanpoiminnan

Syynä on EU:n päätös joustaa lemmikkien maahantuontivaatimuksissa Ukrainan sodan takia.

Eläinten raivotautia on esiintynyt viimeksi 1990-luvun alussa. Se johtaa kuolemaan ilman nopeaa hoidon aloittamista.

Sosiaalisessa mediassa on liikkunut tietoja, joiden mukaan ekinokokin leviäminen johtaisi siihen, ettei loista saataisi enää hävitettyä Suomesta eikä metsästä poimittuja marjoja saisi syödä keittämättä.

Aivan kaikki lemmikit eivät tule viranomaisten tietoon.

Miten ohjeet tavoittavat maahan tulevat ukrainalaiset?

”Se on kysymys, jota täällä on pohdittu. Olemme informoineet eläinlääkäreitä. Ensi kädessä tulijoita on vastassa Migrin henkilökunta, jolle on jaettu ohjeita”, sanoo eläinten terveyden ja lääkitsemisen -yksikön johtaja Sirpa Kiviruusu Ruokavirastosta.
